government–drugmaker direct-negotiation plan would quickly gain Medicaid access
to costly GLP-1 obesity drugs such as Wegovy and Zepbound. Nine months on,
access has narrowed: only Indiana has publicly backed the plan; several states
have stopped reimbursing these drugs for Medicaid patients and others are
considering ending coverage. Most states interviewed say they will not
participate, citing budget pressure and concerns about the long-term fiscal
sustainability of the model, and report other states’ GLP-1 spending has
exceeded expectations by several hundred million dollars.
